Insurance, Fees & Payments

Our clinicians participate with many major insurance plans. Participation varies by clinician and specific plan. Before beginning services, insurance eligibility and benefits can be reviewed so you have better information about applicable copays, coinsurance, deductibles, and other expected financial responsibilities.

Insurance benefit information is based on information provided by the insurance carrier and is not a guarantee of coverage or payment.

Please note that an insurance plan does not guarantee that mental health services will be covered. If you have questions about coverage, you may reach out to your insurance provider at the number usually provided on the back of your insurance card.

Co-pays, coinsurance, and any unmet deductibles are due at the time of each service. NWHRC is unable to provide service to clients who have unpaid fees, unless prior arrangements have been made.

Psychological Assessment Fees

The cost of a psychological assessment varies according to the referral question, complexity of the evaluation, records reviewed, measures administered, time required for scoring and interpretation, and preparation of the written report. Because evaluations differ considerably in scope, a single fee does not apply to every assessment.

When insurance may apply, available benefits will be reviewed before the evaluation begins. Coverage for psychological testing and assessment varies by insurance plan and may be subject to deductibles, coinsurance, authorization requirements, medical-necessity criteria, or other limitations.

Information provided by an insurance carrier is an estimate of benefits and is not a guarantee of coverage or payment. Clients remain responsible for amounts ultimately assigned to them by their insurance plan.

Some specialized evaluations or portions of an evaluation may not be covered by insurance. When this applies, anticipated fees and payment expectations will be discussed before services are provided.

Whenever reasonably possible, clients will be provided with an estimate of the expected scope and financial responsibility before testing begins. If additional assessment procedures become clinically necessary, any meaningful change in the anticipated cost will be discussed before proceeding.

Assessment fees may include time spent on:

  • Clinical interview and history gathering
  • Review of relevant records
  • Administration of psychological or neuropsychological measures
  • Scoring and interpretation
  • Collateral interviews or questionnaires, when appropriate
  • Integration of assessment findings
  • Preparation of the written psychological report
  • Feedback regarding results and recommendations

Clients are responsible for providing accurate insurance information, completing requested assessment materials, attending scheduled appointments, and paying applicable copays, coinsurance, deductible amounts, non-covered services, or other agreed-upon fees.

We encourage clients to ask questions about the anticipated cost of an evaluation before beginning the assessment process. Our goal is to provide clear information about both the clinical scope of the evaluation and the expected financial responsibility before substantial services are provided.

Self-Pay / Out-of-Network

Clients may choose to receive services on a self-pay basis rather than using insurance. Self-pay may also apply when a clinician does not participate with a client’s insurance plan, when a service is not covered by insurance, or when the client elects not to submit a service to insurance.

When services are provided on a self-pay basis, the anticipated fee will be discussed before services begin. Clients are responsible for payment according to the practice’s payment policies.

Whenever possible, we want clients to understand the expected cost of care in advance. If the anticipated scope or cost of services changes meaningfully, this will be discussed before additional services are provided.

Some insurance plans provide reimbursement for services received from an out-of-network provider. Coverage varies considerably by plan and may be subject to an out-of-network deductible, coinsurance, reimbursement limits, or other requirements.

When appropriate, we may provide documentation that clients can submit to their insurance company for possible out-of-network reimbursement. However, submission of documentation does not guarantee that an insurance company will reimburse any portion of the fee.

Clients who wish to use out-of-network benefits are encouraged to contact their insurance carrier before beginning services and ask about:

  • Out-of-network behavioral-health benefits
  • Out-of-network deductibles
  • Coinsurance or reimbursement rates
  • Requirements for prior authorization
  • Procedures and deadlines for submitting claims
  • Whether the proposed service is eligible for reimbursement

Unless another arrangement has been made in advance, clients receiving self-pay or out-of-network services are responsible for payment of the applicable practice fee regardless of whether an insurance company later provides reimbursement.

We encourage clients to ask questions about fees, insurance participation, and expected financial responsibility before beginning services so that payment arrangements are as clear and predictable as possible.
